We are a Georgia\-based licensed home care agency serving both pediatric and senior populations across 20 counties in the Atlanta metro area. Our agency provides skilled nursing, personal care, companion, and in\-home support services through Georgia Medicaid programs, private pay, and long\-term care partnerships.

We are growing aggressively and looking for a highly motivated healthcare marketer and relationship\-builder who can help increase referrals and expand our presence with healthcare professionals, schools, hospitals, senior communities, and community organizations throughout Georgia.

This is NOT a passive admin role. We are looking for someone who enjoys outreach, relationship\-building, follow\-up, and helping grow a healthcare business.

Position Summary

The Home Care Referral Marketing \& Business Development Coordinator will be responsible for generating referral opportunities through consistent outreach, relationship management, follow\-up, and CRM organization.

You will communicate regularly with:

  • Pediatric offices
  • Specialty clinics
  • Hospital discharge planners
  • Case managers
  • Social workers
  • School nurses
  • Assisted living facilities
  • Rehab centers
  • Senior communities
  • Home health and hospice agencies
  • Community healthcare organizations
  • Your primary objective is to help our agency grow referral volume and build long\-term referral relationships across our Georgia service areas.

    Key Responsibilities \- Referral Source Outreach

  • Make outbound calls daily to referral sources across our Georgia counties
  • Introduce agency services professionally and confidently
  • Build relationships with pediatric and senior healthcare referral partners
  • Maintain weekly follow\-up communication with active referral sources
  • Send outreach emails, faxes, and text follow\-ups
  • Relationship Development

  • Build long\-term referral relationships with healthcare professionals
  • Track referral source activity and engagement
  • Identify high\-potential referral opportunities
  • Help strengthen agency visibility within the healthcare community
  • Intake \& Lead Coordination

  • Respond to inbound referrals promptly
  • Conduct warm intake calls with families
  • Help guide families through the onboarding/referral process
  • Coordinate intake appointments and paperwork collection
  • CRM \& Reporting

  • Manage all outreach activity inside Go High Level (GHL) CRM
  • Document calls, emails, notes, and referral status daily
  • Maintain organized referral databases by county and source type
  • Provide weekly reporting on outreach activity and referral progress
  • Growth \& Marketing Support

  • Assist with identifying new referral opportunities
  • Help improve outreach messaging and follow\-up strategies
  • Support marketing campaigns and relationship\-building initiatives
  • Contribute ideas to help grow the agency quickly and effectively

    Strong preference for candidates with experience in:

  • Home care marketing
  • Home health marketing
  • Hospice marketing
  • Healthcare outreach
  • Physician relations
  • Medical office coordination
  • Patient intake
  • Healthcare sales
  • Case management support
  • Community liaison roles
  • Experience with any CRM is preferred.

    Go High Level (GHL) experience is a major plus.

    Requirements

  • Excellent spoken and written English
  • Strong phone communication skills
  • Comfortable making outbound calls daily
  • Reliable internet connection and quiet remote workspace
  • Ability to work independently
  • Available during US Eastern Time business hours
  • Experience using CRM systems and spreadsheets
  • Strong follow\-up and organizational skills
  • Compensation

  • Competitive hourly pay based on experience
  • Performance bonuses tied to qualified referrals and patient enrollments
  • Performance bonus: $50 per qualified referral that is submitted to Medicaid
  • Additional $75 bonus per patient approved and enrolled
  • Flexible remote work environment

  • Pay: $8\.00 \- $15\.00 per hour
