Overview:

- Creates patient Plan of Care per Physician’s orders and ensures compliance
- Implements nursing support for individuals in a home setting
- Reviews nursing documentation to ensure Field Nurse is following Physician’s orders
- Reviewing MARs, verbal orders, medication profile management to match Physicians orders
- Follows policies and procedures according to operation, state and regulatory standards
- Completes supervisory visits based on compliance and regulations to include review of plan of care, caregiver competency assessment, and completion of required documentation
- Training for Caregivers and Nurses as required
- Participates in staff meetings, in\-services and other meetings as required
- Degree in Nursing from an accredited school
- Must have no less than 2 years clinical experience, and at least 1 year in related management/supervisory role
- Current or multi state license in compliance with the state regulations
- CPR professional certification
- Excellent communication skills with all levels of personnel, internal and external to the company
- Capable of working independently with highly confidential information

You’ll play a key role in supervising and supporting caregivers, helping to implement and monitor individualized care plans. This position offers a balance between clinical and administrative responsibilities, allowing you to use your nursing expertise in a way that’s impactful, but without the physical strain of traditional direct care.
