What You’ll Do:
- Develop relationships with clients and their families to provide advocacy and support toward self\-sufficiency.
- Provide case management services with a primary focus on veterans, including housing stabilization, employment support, and barrier reduction.
- Assist veterans in obtaining and maintaining employment through coordination with workforce and training resources.
- Develop and monitor individualized housing and service plans.
- Maintain regular contact with clients, including phone, virtual, and face\-to\-face visits in office, home, and community settings.
- Advocate and coordinate with landlords, employers, service providers, and community organizations.
- Provide referrals to supportive services including healthcare, mental health, substance use treatment, and financial assistance.
- Complete required documentation, including case notes, service plans, and program data entry.
- Attend community meetings and participate in coordinated entry and veteran service networks as directed.
- Ensure compliance with program requirements, including HVRP, SSVF, and housing programs.
- Perform other duties as assigned.
- Bachelor’s degree in social work (BSW), Human Services, or equivalent human services degree; OR
- Registered nurse (licensed)
- This position requires a valid driver’s license, a reliable vehicle, and proof of insurance.
- Experience in case management and homeless/at\-risk of homelessness populations.
- Knowledge of social services and referral sources for families.
- Motivational interviewing skills.
