Hourly Rate: $56,243 \- $65,000 DOE

New hires are typically brought into the role between the minimum and midpoint of the salary range, based on qualifications and experience. Placement above the midpoint is uncommon and generally reserved for internal candidates or individuals with highly specialized experience.

Position Summary

This position will assist and enable an eligible child and family to receive the services and support that have been offered and accepted by the family in accordance with Part C of IDEA. The FRC will help children with special needs and their families reach their full potential. The FRC collaborates with families and the service team to develop a personalized plan that addresses the unique needs of the child and family. All activities will be coordinated with and supervised by the FRC Lead, CDS Director, and Assistant Director.

Essential DutiesResponsibilities

  • Serve as a point of contact in helping parents obtain needed services, funds, and assistance;
  • Case Management (up to 55 active) maintain all documentation of FRC activities in each child's file and collectively for agency records, billing, and tracking. Utilize and enter data on ESIT's DMS as required, and our EHR Credible.
  • Possess strong abilities in ensuring adherence to contract requirements and timelines, which are essential for the program to secure revenue and sustain its operations within Pierce County Contract.
  • Review each child's file monthly to ensure accuracy, parent satisfaction, progress toward outcomes, etc.
  • Review child's progress and performance level with family and provider(s) every 6 months, or as needed.
  • Conduct a transition meeting with school district representatives following policy best practices.
  • Review and facilitate communication regarding the Part B transition, including follow\-up assessments and offering additional resources as appropriate.
  • Co\-facilitate the review of Child Outcome Summary processes at CDS to improve quality, reliability, and validity.
  • Engage in community outreach activities to strengthen relationships, increase referrals, and improve understanding of available resources.
  • + Maintain state training requirements as mandated.

  • Facilitate development, scheduling, and review of Individual Family Service Plans (IFSPs)
  • Coordinate the delivery of therapy and early education services
  • Attend ongoing FRC training opportunities and FRC meetings with HopeSparks Team, County Lead Agency (CLA), and Community Partners as appropriate
  • Work collaboratively with management, educators, and therapists to improve the efficiency and effectiveness of programming
  • Attend the CDS weekly multidisciplinary team meeting
  • Participate in community groups as a representative of CDS as needed or appropriate
  • Must participate in all contractually required trainings as listed on our Pierce County ESIT Contract
  • Must be able to lift 40 pounds (average three\-year\-old).
  • Qualifications

  • BA/BS degree in early childhood or social services strongly preferred
  • Experience working in the field of social services, early intervention, or case management
  • Knowledge of early childhood and special education, specifically related to infants and toddlers' development, developmental delays, and Part C of IDEA
  • Familiar with the child referral, intake, IFSP process, and service delivery
  • Preference given to candidates who have experience working with families from diverse cultures and are fluent in Spanish
  • Strong organizationaltime management skills and demonstrated ability to work efficiently and collaboratively with service providers
  • Strong interpersonal skills to include, but not limited to, a positive attitude, strong listening skills, problem\-solving skills, appropriate professional behavior, and demeanor
  • Proficient English written skills and fluency
  • Ability to represent HopeSparks and the CDS Program within the early intervention and early childhood community in Pierce County
  • Ability to work collaboratively with the Program Director, Assistant Director, Therapists, Educators, and Support Teams, as well as community partners
  • Intermediate or higher level of computer skills; application knowledge in Word, Excel, Outlook Express, and web browsing, data entry, and data management
  • Must have a valid Washington State driver's license, an automobile for on\-the\-job use, and proof of current automobile insurance
  • Have current First Aid/CPR certification
  • Approved DCYF DDA Background Check
  • Job Conditions

    All Family Resource Coordinator services are provided in office, community, and home visit settings. Frequent travel within Pierce County is necessary, and extended or flexible hours are possible.

  • Ability to stand, sit, talk, and listen/hear for extended periods (6\-8 hours) daily in order to perform administrative duties
  • Ability to use hands to finger, handle or write for extended periods (6\-8 hours) daily
  • Ability to reach with hands and arms, and climb or balance to perform filing duties for extended periods (6\-8 hours) daily
  • Ability to ascend and descend stairs to access clients' homes
  • Ability to lift items weighing up to 50 pounds for filing duties
  • Ability to focus and see items with close, distance, color, and peripheral vision, and depth perception to drive, perform computer work, and file (8 hours)

This position is based at the S. Union Office.
