Job Summary:
The Assistant Director of Nursing is responsible for overseeing the proper execution of services for all residents including the successful acclimation of new residents and the continued satisfaction of established residents. This position also provides the direct supervision, coaching, and performance management of the Care Team and is responsible for successful maintenance of compliance with regulations, Standards, and on\-going improvement to execute the company mission and vision.
Duties and Responsibilities:
· On\-going coaching and development of nursing and care staff to ensure quality care delivery and satisfaction
· Assist in the on\-going performance management of nursing and care staff with the oversight of the Wellness Director
· Observing staff to ensure proper delivery of care and services and identify training opportunities for in the moment intervention
· Ensure regulatory and standard compliance and implement interventions
· Monitoring refusals and trends in care
· Preform Skin assessment
· Review daily observations
· Review and maintain pertinent charting
· Review daily ALIS alert reports
· Maintain ALIS documentation to maintain regulatory compliance
· Participates in on call rotation
· Reviews incident reports/post fall evaluation and 24\-hour report following up on any pertinent charting, investigations to give to Director of Nursing.
· Gathers information for weekly nurse call numbers
· Orient new residents/families as needed
· Participates in Care Plan meetings as requested by Director of Nursing.
· Ensure chart audits completed by charge nurses as needed
· Facilitates with Charge Nurses any Home Care or Agency referrals
· Ensures processes of care, orders, family communication, follow up of orders, medication delivery etc. by Wellness Nurse's (both medication and administrative processes) are being followed in wellness offices
· Conduct regular audits of med cart, narcotic book, MARs for quality assurance and regulatory compliance
· Follow up on medication errors and investigation, training opportunities and institute changes
· Publishes draft care plan of new move ins if required
· Maintains aide care lists in ALIS for equal distribution of care/showers. Include input form aides/nurses
· Maintains monthly infection logs, incident logs, skin logs, etc.
· Assist with nursing care on the floor, as needed
· Perform any resident assessments as required by regulation
· Assure all required regulatory medical paperwork is obtained, accurate and complete prior to and/or upon move\-in
· Communicate new residents needs and individual preferences with the Hearth team at move\-in
· Establish new resident care plans or individualized service plans (ISP)
· Coordinate readmissions for current residents assuring all services and necessary documentation is accurate, complete and per regulatory compliance
· Conduct accurate assessments of current Assisted Living residents and update care plan as needed, communicating with Director of Nursing to facilitate next steps of Care Plan Meeting:
o Assessments are scheduled 30 days after move\-in and every 6 months thereafter
o Change of condition assessments as warranted by resident change and in collaboration from Director of Nursing
o Lead and assist in Care Plan meetings in conjunction with Director of Nursing
· Orient and supervise nurses and aides while maintain a positive team morale. To assist in maintain required staffing needs.
· Support the Wellness Aid Program and No Fall Zone
· Attend Marketing events and inter\-department meetings as required
· Assists in maintain on\-call schedule and on\-call responsibilities to include availability for consultation to the nursing staff
· Provide coverage and/or on call for absences of the Wellness Director or other Case Managers
· Assist with supervision of nursing staff through participation in orientation, staff meetings and required in\-services
· Follow Management’s Policies and Procedures
· Assist in maintaining all required reporting logs
· Provide support to the Live More and Think More Philosophies
· Any other job duties as assigned by Director of Nursing
Qualifications:
A valid LPN license is required with two (2\) years management experience in long\-term care, assisted living, CCRC, or other senior housing related field or equivalent experience. Flexibility in work schedule is key in order to respond to urgent issues as they arise after hours. Requires excellent Verbal, Written and Interpersonal Skills. Proficiency in Microsoft Office applications such as Word, Excel, Outlook and similar technologies. Must have advanced working knowledge of state department of heath regulations and must have general working knowledge of employment regulations including DOL, Hour and Wage, and EEOC.
Skills and Abilities:
Moderate reading, writing, grammar, and mathematics skills; proficient interpersonal relations and communicative skills; auditory and visual skills; ability to bend, stoop, sit, stand, reach, and lift items weighing 50 pounds or less; satisfactory completion of post\-offer, pre\-employment physical as required.
