Supports Coordination Assistant
Position Overview: We are seeking a detail\-oriented Supports Coordination Assistant to provide essential support to our coordination team. In this role, you will perform many of the same core tasks as a Supports Coordinator, such as linking clients to resources, processing referrals, and conducting check\-ins, all under the direct supervision of a licensed Supports Coordinator. - Assist with linking clients to community resources and support services under supervision.
- Process and track referrals to ensure timely service initiation.
- Conduct regular check\-in calls and visits with clients to monitor well\-being and service utilization.
- Gather and update client information for care plans and assessments.
- Coordinate communication between clients, families, and service providers.
- Assist Supports Coordinators with documentation and data entry in compliance with MDHHS regulations.
- Support the preparation of individualized service plans (ISPs) by collecting necessary data.
- Respond to client inquiries and escalate urgent needs to the supervising Supports Coordinator.
- High School Diploma or GED required.
- 1\+ year of experience in social services, case management support, or healthcare administration.
- Strong knowledge of Michigan community resources and service systems.
- Excellent communication and interpersonal skills.
- Ability to work effectively under the supervision of a licensed Supports Coordinator.
- Valid Michigan driver's license and reliable transportation.
- Proficiency in Microsoft Office and electronic documentation systems.

- Experience with Michigan Medicaid waiver programs
- Previous experience conducting client check\-ins or home visits.
