- Client Management: Carry an active caseload of eligible clients, providing direct one\-on\-one medical case management services. Able to provide services in\-person, over the phone and on a drop\-in basis.
- Assessments \& Planning: Conduct health assessments and goal planning in collaboration with the client to identify medication adherence plans and barriers to care. Develop individualized care plans based on assessments and available resources to improve medication adherence and overall health.
- Coordination of Services: Work collaboratively with case management team, providers, community partners to facilitate access to HIV treatment and medical care. Provide client education pertaining to HIV and other medications, disease progression, and risk reduction.
- Advocacy: Advocate for clients with medical service providers and case managers. Conduct case conferences with case managers and medical providers as needed.
- Documentation: Maintain accurate and timely documentation of client interactions, services provided, and progress updates in agency databases. Process and collect necessary eligibility documents including Authorization for Release of Information.
- Referrals: Refer clients to infectious disease providers and medical resources as needed and follow up.
- Community Engagement: Build partnerships with medical providers in our service region. Attend relevant community meetings and participate in ongoing training and professional development.
- Travel: Ability to travel to see clients in other community settings and provide home visits on occasion. Ability to transport clients on occasion.
- Standards: Comply with all applicable program standards, guidelines, laws and administrative rules.
- Reporting: Assist management with all necessary reports.
- Ability to work well with others, handle stress appropriately, have reliable attendance
- Valid Oregon Registered Nursing license
- One year of experience providing clinical care or medical case management to patients with chronic disease or infectious disease
- Ability to pass AIDS RN Certification within 24 months
- TB test (provided at agency)
- Valid Oregon driver’s license, or valid out of state driver’s license with the ability to get an Oregon driver’s license within 30 days of establishing residency in Oregon, and a driving record sufficient to be covered by agency auto insurance policy.

- Use continuous clarity of vision at 20 inches or less for processing of paperwork
- Ability to work flexible hours, including early mornings, evenings, and weekends
- Ability to work a minimum of 4 days per week for 30 hours per week

Position Summary: This position will provide medical case management services to clients enrolled in Ryan White Case management program. The HIV Medical Case Manager will communicate with providers regarding the client’s care plan and the medical treatment plan. In addition, HIV Medical Case Managers will provide services such as: treatment management and medication adherence, HIV disease and side effects management, nutritional therapy, oral health interventions and referral, liver health and hepatitis disease management (acute and chronic) and HIV risk reduction.
Key activities include: initial assessment of service needs; development of a comprehensive, individualized care plan; provision of services required to implement the plan; monitoring to assess the efficacy of the plan; and periodic re\-evaluation of the plan as necessary. This includes all types of case management including face\-to\-face, phone contact, and any other form of communication.
