FUNCTION:
As a member of the rehabilitation team, the Social Services Assistant supports the Social Services Director in providing services to respond to the emotional needs of Residents and their families. This employee also assists with resident admissions, care conferences, and the referral process. Skilled Nursing Experience Preferred.
RESPONSIBILITIES:
- Support the Social Services Department by completing administrative/clerical duties.
- Assist with obtaining consent forms and signatures from residents/families for medical release forms and services.
- Gather pertinent information from residents/families as directed by the Social Services Director/Social Worker.
- Provide support during discharge planning, referrals to home health care, hospice, other long term care/assisted living facilities, etc.
- Fax, scan, copy, email and file documents, as well as answer phone calls.
- Adhere to confidentiality requirements, HIPPA.
- Understand and demonstrate knowledge of and respect for the rights, dignity and individuality of each resident in all interactions.
- Demonstrate honesty and integrity at all times in the care and use of resident and facility property.
- Demonstrate respect for co\-workers and responds to needs of residents by complying with facility policies.
- Demonstrate ability to prioritize tasks and responsibilities and complete duties within allotted time.
- Demonstrate willingness to take risks, generate new ideas for change, evaluate and recognize priorities, challenge others to learn, keep current and integrate new information.
- Communicate and model organization values, foster high performance, recognize need for and provide adequate resources to residents and fellow staff.
- Perform other duties as assigned.
- Ability to ask for help when an assignment is not understood.
- Working knowledge of personal computer and software applications used in job functions, (word processing, graphics, databases, spreadsheets, etc.)
- Strong organizational and analytical skills; oral and written communication skills.
- Current knowledge of state and federal laws and regulations that apply to practice of social work in long\-term care.
- Bachelor’s Degree in Social Work or Psychology, or two years supervised social work experience working directly with individuals in a long\-term or sub\-acute care rehabilitation is preferred.
