Position Summary and Job Duties

  • Work involves assisting Nutrition Program Supervisor in day to day administration of the congregate and home delivered meal programs. This position also supports and promotes the Mission, Vision and Guiding Principles of the Human Services Department and performs in a manner consistent with the HSD Professional Standards and "Commitment to My Co\-Workers."

    85%\- Assist Nutrition Program Supervisor in maintaining all activities of the Nutrition Program

  • Assist with training of congregate and home delivered meal volunteers.
  • Attend regularly scheduled meetings with dining center managers to meet training requirements and regularly visit dining sites.
  • Assist in maintaining appropriate record\-keeping documents to ensure proper data reporting.
  • Distribute supplies to dining centers as needed.
  • Assist in conducting an annual participant satisfaction survey.
  • Assist in collecting and organizing documentation as required for the assessment of the Elder Nutrition Program.
  • Assist as needed in initial intake process to ensure participant is eligible for the program and conduct regularly scheduled reassessment visits.
  • Make appropriate referrals to meet other needs participant may have.
  • Assist in referring "at nutritional risk" individuals to contracted dietitian for individual consultation.
  • Assist with daily meal ordering as needed.
  • Produce documents for publication such as menus, nutrition education and others.
  • 5%\- Assist Supervisor as needed with administration of the annual Senior Farmers' Market Nutrition Program, including the following tasks:

  • Arrange for and schedule locations for voucher distribution and assist with those distributions.
  • Promote the voucher program.
  • Train volunteers in distribution requirements.
  • Oversee distribution sites and/or mailings.
  • Maintain required records and complete all required reports.
  • 5% \- Provide backup and support for Site Managers and Home Delivered Meal Drivers

  • Provide back up to cover Rock County Nutrition Program Dining Sites.
  • Provide back up to deliver home delivered meal routes.
  • 5%\- Other duties as assigned.

  • Job Requirements, Education, Training \& Experience

  • High School diploma required; Bachelor's degree from an accredited college or university preferred.
  • Course work, training and/or experience which demonstrates the knowledge, skills and ability to effectively perform the functions of this position.
  • Experience working with the older population preferred.
  • Serving Safe Foods Certification required at time of hire or immediately thereafter.
  • Knowledge of the Older Americans Act Elder Nutrition Program desirable
  • Familiarity with non\-profit and human services agencies in Rock County desirable.
  • Ability to work independently and as part of a team.
  • Ability to work weekends and evenings on occasion.
  • Must have valid driver's license and access to a reliable personal vehicle for business\-related travel.
  • Occasional out\-of\-county travel may be required.
  • Knowledge of and proficiency in various computer software programs, especially Microsoft Word and Excel.
  • Experience doing mass mailings, including merge documents.
  • Essential Job Functions (physical elements, equipment and working conditions)

  • Ability to sit, walk and stand for long periods of time.
  • Ability to lift up to 50 pounds.
