Wellness Homes of Chicago (“WHoC”) is seeking a Patient Navigator to join our growing, dynamic and diverse multidisciplinary healthcare group with three (3\) locations across the Chicago metro area. This position is based at our Halsted location in Bridgeport.
The Patient Navigator will work closely with the Medical Case Manager (MCM) to assist patients and clients in accessing medical care, navigating insurance, and connecting with vital community resources. This position offers a valuable opportunity to gain experience in the healthcare field and make a real difference in the lives of vulnerable individuals. This position requires an understanding of the healthcare system, excellent communication skills, and a genuine passion for helping vulnerable populations.
#### Navigator Responsibilities:
- Support the MCM in scheduling and confirming medical appointments for patients.
- Assist with arranging transportation as needed.
- Update and maintain patient appointment records.
- Help patients gather necessary information for insurance applications and claims.
- Assist with insurancerelated inquiries and paperwork under the guidance of the MCM.
- Assist patients with navigating prescription cost savings or alternative programs to obtain medication(s).
- Educate and help familiarize qualified patients with the Illinois ADAP/Ryan White Foundation.
- Equipment inventory management.
- Submit insurance verification for approval.
- Complete FMLA and patientrelated documentation to support patients.
- Inventory management and assist MCM with new equipment installation(s).
- Support the MCM in coordinating care between patients, internal staff, and external healthcare providers.
- Assist with referrals to specialists and external resources.
- Assist with housing referral applications and followup.
- Connect patients with social services and benefits programs.
- Assist with the management of the Remote Patient Monitoring (RPM) and Chronic Care Management (CCM) programs.
- Data entry, patient communication, and technology troubleshooting under the direction of the MCM.
- Participate in community events and health fairs alongside the MCM.
- Provide administrative support to MCM, including data entry, filing, and maintaining patient records.
- Other jobrelated duties as assigned.
- Associate Degree in Human Services, Social Work, or a related field preferred.
- Experience working with vulnerable populations or in a healthcare setting.
- Basic understanding of healthcare systems and insurance plans.
- Excellent communication, interpersonal, and organizational skills.
- Proficiency in Microsoft Office Suite, EMR and similar software.
- Ability to work independently and as part of a team.
- Must be able to work flexible hours, including some weekends and attend community events.
- A genuine desire to help others and contribute to their wellbeing.
- Reliable, dedicated, and eager to learn.
- Ability to work collaboratively and be a team player.
- Excellent communication skills.
- Accuracy in handling information and completing tasks.
- Valid driver's license and reliable transportation.
