Virtual support assistant role to support front line clinical staff in care of patients receiving palliative care in the home setting. Serve as an extension to front line staff in patient care logistics management including patient outreach, consent, scheduling. Responsible for coordinating with clinical staff to ensure scheduling efficacy, provider orders carried out, delivered, and vendor interfaces successful for order completion. Support care of home based census of patients. Patient outreach, program education, consent and scheduling for enrollment Works as a member of our interdisciplinary care team assessing and addressing the complex care needs of patients and families Supports admission and ongoing home based census operations and follows up on orders to coordinate care Provides administrative support for tracking enrollments and ongoing care activities (eg Appointment scheduling, discharge orders, hospice transitions, etc) Communicates plan of care and assessments to front line teams Participates in huddles/IDT meetings daily/weekly with the rest of the care teams Strong organizational and communication skills Documentation as required Performs other related duties as assigned Required: High school diploma or equivalent. 1\+ years healthcare experience Facility with online ordering systems, data entry Ability to work both day and evening shifts, rotating holidays, and on call Preferred: Associate degree ideally in healthcare Spanish speaking proficiency/fluency Experience working with EMR, strong telephonic and coordination organizational skills Compensation Statement The Mount Sinai Health System (MSHS) provides salary ranges that comply with the New York City Law on Salary Transparency in Job Advertisements. The salary range for the role is $19\.02 \- $28\.53 Hourly. Actual salaries depend on a variety of factors, including experience, education, and operational need.
