When an older adult receives a complex diagnosis or leaves a care facility, the crisis is rarely just medical. It is administrative. Families are instantly buried under confusing insurance denials, endless phone trees, expensive medical bills, fragmented provider schedules, missing referrals, unclear next steps, and disconnected care teams. They are forced to become project managers during some of the most stressful moments of their lives. Prevail is fixing this. Prevail is a patient advocacy platform that connects older adults to healthcare advocates. We give Medicare patients and their families their own healthcare advocate, someone who helps them understand what needs to happen next, coordinates across providers, follows up on open issues, organizes care tasks, and makes sure patients do not fall through the cracks. We are an early\-stage, mission\-driven startup, led by experienced healthcare founders and previous startup builders looking to using technology and healthcare experience to build the logistics and navigation gap between complex medical systems and the $1\.4 trillion Medicare market. About the Role Prevail is seeking experienced, independent Healthcare Advocates to provide healthcare navigation and advocacy services to Medicare patients through the Prevail platform. You have seen patients become lost between providers, referrals, discharge instructions, pharmacies, insurance plans, senior living communities, home health agencies, and family members. You understand that the difference between a patient receiving meaningful support and falling through the cracks is often one person who takes ownership, creates a plan, and follows through. This is not a traditional call\-center opportunity. This is not bedside nursing. This is not a role where every patient situation comes with a perfect playbook. It is an opportunity for experienced healthcare navigators who combine empathy with execution, patience with urgency, and healthcare knowledge with exceptional follow\-through. You should be the kind of person who sees a complicated patient situation and thinks: "I can figure this out." Services You May Provide Serve as the patient\-facing point of contact following an intake, diagnosis, discharge, or care transition Build strong, trusting relationships with Medicare patients and their families Develop and maintain patient navigation plans with clear needs, next steps, responsible parties, and timelines Coordinate appointments with primary care providers, specialists, imaging centers, pharmacies, home health agencies, therapy providers, and community organizations Help patients work through referrals, insurance questions, benefits, forms, provider communication, and follow\-up tasks Help patients access and use healthcare portals, scheduling systems, and telehealth tools Identify practical barriers related to transportation, food access, caregiving, medication access, home safety, and benefits, and locate community resources to address them Communicate with patients, caregivers, providers, facilities, pharmacies, insurers, and community organizations Document patient interactions, open issues, completed activities, and follow\-ups through the Prevail platform Recognize when a concern requires clinical review and escalate it to an appropriate clinical professional Follow through on accepted patient commitments until the matter is completed, transferred, or appropriately escalated Scope of services: Advocates provide healthcare navigation, coordination, and non\-emergency advocacy services. Advocates do not diagnose medical conditions, prescribe medications, provide medical treatment, replace a patient's physician, make clinical decisions on behalf of a treating provider, or provide legal, tax, or financial advice. Emergencies should be directed to 911, and medical concerns escalated to the patient's licensed healthcare provider as well with notifying the Prevail team right away. Who This Is For At least 3 years of experience in healthcare advocacy, care coordination, case management, patient navigation, Medicare support, home health, discharge planning, nursing, benefits navigation, or social services Experience working with Medicare patients, older adults, caregivers, or medically complex populations You can take an unclear patient concern and turn it into a practical action plan, and you are not intimidated by incomplete information You communicate with older adults and caregivers using patience, empathy, and clarity, and you are comfortable calling physician offices, senior living care teams, pharmacies, insurers, facilities, and equipment suppliers Excellent verbal communication over phone and video, with strong written documentation habits Highly organized and capable of managing multiple patients, open issues, and follow\-ups without losing important details Sound judgment: you know when a concern can be handled through navigation and when it should be escalated to a clinical professional Comfortable working independently, managing your own time, and using modern technology platforms Most importantly, you take ownership of the commitments you choose to accept Why RN Experience Is Valuable An active Registered Nurse license is not required, but RN experience is strongly preferred. Experienced RNs bring deeper understanding of medical terminology, medications, and discharge instructions; credibility communicating with physicians and interdisciplinary care teams; strong patient education skills; and the judgment to recognize warning signs that should be escalated. Preference may also be given to: LPN/LVN license in good standing, Certified Case Managers, Board Certified Patient Advocates, social work education or licensure, community health workers, medical assistants with care coordination experience, Medicare Advantage or care management experience, senior living or geriatric care experience, and fluency in Spanish or another language commonly spoken by Medicare populations. The most important qualification is a demonstrated ability to help patients navigate healthcare problems effectively, independently, and compassionately. Compensation Independent Healthcare Advocates may propose or negotiate an hourly rate for services provided through the Prevail platform. Agreed\-upon rates generally range from $25 to $45 per hour, depending on advocacy experience, professional credentials, Medicare and senior care experience, language capabilities, availability, and the scope of services accepted. Prevail compensates advocates for approved service time associated with accepted patient opportunities, including related documentation and communication. Prevail does not guarantee any minimum number of patients, assignments, hours, or compensation, Availability and Remote Setup You control your own availability. Advocates who offer at least 20 hours of weekly availability across three or more weekdays, during standard healthcare office hours, are best positioned to receive patient opportunities and provide continuity of support. This opportunity is fully remote. All interactions occur by phone, video call, and secure messaging. You should have reliable internet, a reliable phone and computer, a private and professional workspace, and the ability to protect confidential patient information. You must be authorized to provide services in the United States.
