Opened in December 2024, we provide compassionate, high\-quality care including companion care, personal care, memory care, cancer care support, and the CMS GUIDE Program in partnership with Pocket RN. We are a growing, mission\-driven agency looking for a skilled and personable Clinical Liaison \& Care Coordinator to help us connect with the healthcare community and support our clients from first contact through ongoing care. Role Overview The Clinical Liaison \& Care Coordinator serves as the bridge between SYNERGY HomeCare and the broader healthcare community. You will build and maintain referral relationships, support client intake and assessments, and handle key administrative functions that keep the office running smoothly. This is a visible, relationship\-driven role that requires both clinical credibility and strong organizational skills. Key Responsibilities Community Outreach \& Referral Development Build and maintain relationships with referral sources including hospital discharge planners, skilled nursing facilities, physicians, social workers, and case managers Represent SYNERGY HomeCare at community events, health fairs, and networking opportunities Conduct regular outreach visits to referral partners in the Howard County area Track and follow up on referral leads to convert inquiries into active clients Client Intake \& Care Coordination Conduct initial client consultations and home visits to assess care needs Collaborate with the care team to develop individualized care plans Serve as a clinical resource for caregivers, clients, and families with questions about care Support transitions of care from hospital, rehabilitation, or skilled nursing settings to home Administrative Support Answer incoming calls and respond to client and family inquiries Maintain accurate client records and documentation Assist with scheduling coordination and caregiver\-client matching Support the owner with operational tasks, reporting, and internal communications Help maintain compliance with agency policies and state home care regulations What We're Looking For Registered Nurse (RN), Licensed Practical Nurse (LPN), Licensed Clinical Social Worker (LCSW), or degree in Healthcare Administration or related field Experience in home care, hospital case management, discharge planning, or community health preferred Strong relationship\-building and communication skills — comfortable engaging with healthcare professionals at all levels Organized, self\-motivated, and able to manage multiple priorities independently Compassionate and client\-centered approach to care Proficient in Google Workspace or Microsoft Office; familiarity with home care software is a plus Valid driver's license and reliable transportation (local travel required) What We Offer Competitive compensation commensurate with experience Flexible scheduling options (full\-time or part\-time considered) Opportunity to shape the clinical identity and community presence of a growing agency Supportive, close\-knit team culture where your work makes a direct impact Mileage reimbursement for local travel SYNERGY HomeCare of Ellicott City\|Ellicott City, MD\|osat.onestepatatime@gmail.com*
