We are currently seeking a compassionate, organized, and experienced Registered Nurse (RN) to join our growing Pediatric Home Care team. This position combines clinical oversight, caregiver education, family support, and quality assurance to ensure the highest standard of care for our pediatric clients. In this advanced role, you will review nursing documentation, caregiver charting, physician orders, care plans, and clinical records to ensure compliance with agency standards, Medicaid requirements, accreditation standards, and quality assurance initiatives. Position Summary ==================== The Registered Nurse (RN) serves as the clinical leader for assigned pediatric clients receiving skilled nursing and personal care services in the home. The RN conducts assessments, develops individualized care plans, supervises caregivers and nurses, provides clinical education, ensures regulatory compliance, and partners with physicians and families to deliver outstanding patient\-centered care. Primary Responsibilities ============================ Clinical Care Conduct comprehensive pediatric nursing assessments for medically fragile children. Develop individualized Plans of Care based on physician orders and patient needs. Initiate, update, and revise care plans as the child's condition changes. Monitor patient progress and communicate changes to physicians and interdisciplinary team members. Ensure continuity of care while maintaining the highest clinical standards. Participate in admission, recertification, reassessment, and discharge planning. Caregiver \& Nursing Supervision Supervise Licensed Practical Nurses (LPNs), Certified Nursing Assistants (CNAs), Home Health Aides (HHAs), Personal Care Aides (PCAs), and caregivers providing services. Conduct routine in\-home supervisory visits according to Maryland regulations and agency policy. Observe clinical skills and provide coaching, mentoring, and competency evaluations. Conduct caregiver orientation and ongoing clinical education. Recognize outstanding employee performance while promoting professional growth. Family Support Educate parents and caregivers regarding disease processes, medications, treatments, equipment, emergency procedures, and care techniques. Respond professionally to family concerns and resolve issues promptly. Build trusting relationships with children, families, physicians, hospitals, and referral partners. Quality Assurance \& Compliance Ensure compliance with all federal, state, Medicaid, accreditation, and agency regulations. Participate in Quality Assurance and Performance Improvement (QAPI) initiatives. Review documentation for accuracy, completeness, and regulatory compliance. Participate in clinical audits and chart reviews. Maintain confidentiality and HIPAA compliance at all times. Administrative Responsibilities Maintain accurate clinical documentation within the agency's electronic medical record system. Complete supervisory visit documentation within required timeframes. Communicate effectively with physicians, case managers, therapists, referral sources, and interdisciplinary team members. Participate in staff meetings, continuing education, and agency training. Qualifications ================== Required Active Registered Nurse (RN) license in the State of Maryland. Minimum one year of nursing experience (pediatric or home health experience preferred). Strong assessment and clinical decision\-making skills. Excellent communication and documentation skills. Current CPR Certification. Current TB testing. Valid driver's license with reliable transportation. Ability to travel throughout assigned service areas. Ability to work independently while collaborating with an interdisciplinary healthcare team. Preferred Experience Pediatric Home Care Private Duty Nursing Medically Fragile Children Trach Care Ventilator Care Feeding Tubes (G\-Tube/J\-Tube) Medication Administration Seizure Management Home Healthcare Medicaid Waiver Programs Electronic Medical Records (EMR) Physical Requirements ========================= Ability to stand, walk, bend, lift, and assist patients as necessary. Ability to communicate effectively verbally and in writing. Ability to operate computers, tablets, and office equipment. Ability to safely travel to patient homes throughout assigned territories. Ability to meet all agency physical, background, drug screening, and health requirements.
