Overview
We are seeking a dedicated and compassionate Counselor \- II to join our dynamic behavioral health team. In this role, you will provide comprehensive mental health services, including psychotherapy, crisis intervention, and case management, to individuals and families facing diverse challenges. Duties
- S/he will implement and facilitate Individual, Couples, Family and/or Group Counseling and Educational services to patients.
- S/he will complete a comprehensive, patient centered biopsychosocial assessment and formulate diagnostic impressions to guide treatment planning, in a timely manner.
- S/he will review referrals to screen for the appropriateness and readiness of an applicant for treatment.
- S/he completes a Psychosocial Summary following Policies and Procedures on all patients applying for Outpatient treatment. The Counselor will submit a Psychosocial Summary that is completed in a timely manner.
- S/he ensures that urine samples are collected under supervision by themselves or another same‑sex staff member.
- S/he assures that urine specimens are appropriately labeled and ready for lab collection/submission.
- S/he complies with all DMHAS,Department of Public Health regulations and all other applicable regulatory agencies pertaining to all levels of care.
- S/he complies with all Connecticut Counseling Centers Policies, Procedures and regulations regarding the treatment of patients.
- S/he maintains charts for each patient, which will include admission data, progress notes per regulatory requirements, treatment plans, and all other forms and data applicable to each patient.
- S/he reports weekly to the Program Director or designee to review progress and patient treatment needs.
- S/he ensures that patient fees from self\-paying patients are collected and coordinates with the Administrative Assistant to confirm that all payments are accurately recorded.
- S/he will maintain a current CT Licenseat all times, by participating in continuing education and staff development.
- S/he will perform community liaison work when requested by the Program Director to inform the community of the services that Connecticut Counseling Centers, Inc. offers and to maintain full census.
- S/he will ensure completion of data entry of patient information into DDaP system per CT State requirements.
- S/he develops a positive therapeutic working relationship with patients including modifying interventions to meet the values and expectations of the patients.
- S/he will consistently demonstrate the ability to utilize best practices in crisis management techniques to effectively intervene, de\-escalate and resolve patient crisis.
- S/he will work closely with collaborating counselors and/or the CT Department of Justice Court Support Services Division (CSSD). S/he will keep all referring staff updated on changes in patient’s treatment status
- S/he will complete, in entirety, ASAM criteria to guide treatment planning and service delivery/referrals for individual care.
- S/he will perform additional duties as assigned.
- CT Licensure with at least one of the following; LADC, LCSW, LPC, LMFT, LMSW, LPCa, LMFTa
- Proven experience in home \& community care settings, hospital environments, or behavioral health clinics.
- Strong skills in psychotherapy, motivational interviewing, addiction counseling, and diagnostic evaluation.
- Knowledge of crisis intervention techniques, PTSD care protocols, and behavior management strategies.
- Familiarity with medical records management, CPT coding (Current Procedural Terminology), ICD coding systems (ICD\-9/10\), and medical documentation standards.
- Experience with intake processes, patient assessment tools, and clinical counseling methods such as cognitive behavioral therapy (CBT).
- Ability to handle sensitive situations with professionalism while adhering to HIPAA guidelines for patient privacy.
