Nurse Practitioner – Geriatric Home\-Based Primary Care (Growth \& Leadership Track)
Location: Greater Bangor, Maine area (home visits \+ optional telehealth)
Schedule: Flexible (part\-time to full\-time)
About the Role
Established, growing home\-based primary care practice seeking a Nurse Practitioner interested in building and shaping a role, not just filling one.
This is a partner\-style position (not ownership) with high autonomy and the ability to grow a clinical focus area within a small, innovative practice.
What You’ll Do
- Provide home\-based primary care to geriatric patients
- Manage complex chronic conditions and transitions of care
- Coordinate with families, caregivers, and services
- Contribute to workflow design and care model development
- Option to lead or expand a program (CCM, dementia care, palliative, etc.)
- Collections\-based earnings tied to your patient panel
- Shared infrastructure instead of full overhead, including:
- EHR, billing, admin support (\~9% of collections)
- Dedicated remote Medical Assistant (\~$4,000/month, shared)
- Net income scales with volume and program growth (CCM, AWV, TCM, etc.)
- Interest in geriatrics, home\-based care, or complex patients
- Independent, self\-directed, and adaptable
- Entrepreneurial mindset; interested in growing services over time
- Strong communication and clinical judgment
