Responsibilities
- Conduct comprehensive patient assessments, including vital signs, physical examinations, and detailed health histories to inform individualized care plans.
- Administer medications and injections with precision, including IV infusions and medication management aligned with HIPAA regulations and medical documentation standards.
- Operate EMR (Electronic Medical Records) and EHR (Electronic Health Records) systems such as Epic, Cerner, Athenahealth, or eClinicalWorks to document patient data accurately and efficiently.
- Provide specialized nursing interventions such as wound suturing, ostomy care, tube feeding management, ventilator support, and sterile processing procedures.
- Collaborate with multidisciplinary teams to coordinate patient care plans for diverse populations including pediatrics, geriatrics, behavioral health patients, and individuals with developmental disabilities.
- Lead triage assessments in urgent care or emergency settings to prioritize patient needs based on acuity levels like Level I Trauma Center or Level II Trauma Center experience.
- Support discharge planning and case management efforts to ensure seamless transitions from hospital to home or long\-term care facilities.
- Maintain strict adherence to infection control protocols and aseptic techniques during all procedures to ensure patient safety.
- Valid RN license in the state of employment with current CPR/BLS certification.
- Proven experience in critical care environments such as ICU or emergency medicine; trauma medicine experience preferred.
- Familiarity with EMR/EHR systems like Epic or Cerner; proficiency in ICD\-9/ICD\-10 coding and medical terminology is highly desirable.
- Demonstrated ability to perform advanced nursing skills including IV insertion, phlebotomy, suturing, catheterization (including Foley and other types), and specimen collection/processing.
- Experience working with diverse patient populations such as pediatrics (including neonatal ventilator management), geriatrics (memory care and dementia), behavioral health, or hospice \& palliative medicine.
- Knowledge of occupational health protocols, infectious disease care standards, pain management strategies, and post\-acute care practices.
- Strong communication skills for effective patient assessment and health coaching; ability to work independently and within team environments across outpatient clinics or inpatient units.
