We deliver 24/7 virtual first and in home medical, behavioral, and social care, working alongside families, providers, and health plans to break down barriers to quality care. We do not replace existing care teams; we enhance them, providing an extra layer of support with compassion, creativity, and an unwavering commitment to children with medical complexity.
*The primary location of this position is remote with an expected schedule of Monday \- Friday (8:00am\-5:00pm).*
What You'll Do
The Care Team Assistant Manager, Transitions of Care, will oversee the Care Team Assistants and Care Coordinator roles responsible for supporting our Transition of Care team. This role is largely administrative in that it oversees a non\-clinical support team. These roles download and scrub data, prep charts for providers, track caseloads, request records, and provide general administrative support to several teams. This team will work closely with Transition of Care Managers. In this role, you will:
- Manage a team of non\-clinical roles focused on transitions of care support.
- Uphold team members to Imagine specific policies, clinical programming requirements, and utilization targets.
- Provide 1:1 supervision to team members to review both administrative topics.
- Organize and lead departmental team meetings inclusive of attendance, agenda, and meeting minutes.
- Evaluate care team performance quarterly and hold members accountable for KPIs and OKRs.
- Develop workflow and productivity requirements.
- Create strategies and workflow to scale the program.
- Work collaboratively with all other leadership roles to respond to feedback, improve patient experience, and support the development of new programs and services.
- Ensure that data is properly downloaded and tracked.
- Identify and participate in cross departmental quality improvement efforts.
- Identify and participate in strategic efforts to best support the service line's operational needs.
- Communicate progress, needs, and updates to leaders, escalate issues when necessary.
- Performs other duties as assigned.
- At least 3 years of experience in people leadership.
- Previous experience in data management.
- Extremely strong Microsoft suite knowledge.
- Ability to maintain and track multiple data sets.
- Understanding of healthcare and/or experience working in a healthcare environment.
- Experience managing a team in a virtual environment.
- Passion about the growing role innovative technology can play in improving access to care and care delivery.
- Be an independent self\-starter, a leader, and a strategic thinker who is excited about the big picture of whole health, and the ongoing evaluation and iteration of our care model.
- Experience working within an interdisciplinary team.
- Comfort working in a tech\-heavy environment.
- A stable and reliable internet connection, can troubleshoot basic tech issues and you have a private place to work.
- Bilingual Spanish highly preferred

What We Offer (Benefits \+ Perks)
This role offers a base salary range of $75,000 \- $95,000 in addition to competitive company benefits package and eligibility to participate in an employee equity purchase program (as applicable).
