Job Description
The Clinical Forms Analyst supports the enterprise\-wide management of clinical forms and documentation integrity initiatives under the direction of the Enterprise Clinical Supervisor, Forms Management \& Documentation Integrity. This role is responsible for coordinating intake, design, build support, version control, tracking, and maintenance of electronic and paper clinical forms across the organization. The Analyst collaborates with clinical departments, HIM, Clinical Informatics, Epic analysts, UAB Print Shop, Regulatory, Legal, Risk, Compliance, and scanning teams to ensure forms are accurate, compliant, standardized, and aligned with clinical workflows and documentation integrity standards. The role ensures timely processing of form requests and maintains detailed governance documentation to support audit readiness and regulatory compliance. This position serves as an operational subject matter resource for clinical forms and supports workflow efficiency, downstream data integrity, and enterprise governance standards
Key Duties \& Responsibilities
1\. Coordinates intake and tracking of all clinical form requests, ensuring completeness of submission, appropriate prioritization, and documentation of approval workflows.
2\. Assists in the development, formatting, standardization, and maintenance of electronic and paper clinical forms in alignment with enterprise governance standards.
3\. Supports Epic build and modification activities related to clinical forms in collaboration with Epic analysts and Clinical Informatics teams.
4\. Maintains enterprise forms inventory, including version control, metadata, approval history, retirement documentation, and audit records.
5\. Performs routine reviews of existing forms to identify outdated, duplicative, or non\-compliant content and escalates findings to the Supervisor.
6\. Ensures forms meet HIM documentation standards, regulatory requirements, downtime procedures, and scanning specifications.
7\. Partners with MRG scanning teams to validate format, legibility, and downstream document indexing requirements.
8\. Supports testing and validation of new or revised forms prior to production release.
9\. Communicates project status, timelines, and requirements clearly to requestors and stakeholders.
10\. Assists with audit preparation activities related to documentation integrity and forms governance.
11\. Participates in enterprise forms governance meetings and prepares supporting documentation or reports.
12\. Provides user support and basic education to clinical departments regarding form standards and submission processes.
13\. Identifies opportunities for workflow improvement, standardization, and efficiency in forms processes.
14\. Performs other duties as assigned
