Position Summary
As an I\-START Coordinator, you will play a vital role in our program. While this position allows for some remote work, it is essential to note that you will be traveling extensively throughout the service area to provide hands\-on support and intervention.
Job Responsibilities
- Regular Travel: Expect to travel frequently within the service area, conducting outreach visits to individuals’ homes, day programs, and vocational settings.
- Active Caseload Management: Maintain an active caseload of individuals referred for services, ensuring timely coordination of support meetings and crisis plan development.
- Clinical Assessment: Utilize clinical documents and assessments to inform case planning and strategy development.
- Community Training: Develop and present training materials to community stakeholders as assigned.
- Crisis Response: Participate in on\-call crisis response and assessment on a rotational basis, providing immediate support as needed.
- Documentation: Maintain thorough clinical documentation and complete data entry tasks within established timelines.
- Professional Development: Attend the Center for START Services Coordinator course within the first year and participate in ongoing training and certification renewal.
- Education: Master’s Degree in Social Work, Psychology, Counseling, or a related human service field (2 years’ experience with ID and mental health) OR Registered Nurse or Bachelor’s Degree in Social Work, Psychology, Counseling, or related field (4 years’ experience with ID and mental health).
