Partners In Home Care (PIHC), Western Montana’s leading non\-profit home health provider since 1987, is seeking a compassionate and skilled Intake Nurse Liaison to join our team. In this role, you’ll coordinates referrals, admissions, medical orders, insurance verifications, and other services for PIHC programs.
This position serves as a pivotal point in communication with referral sources and service areas to ensure efficient processing of referrals and service delivery. Furthermore, this position is responsible for conducting activities which expedite patients’ transition from external care settings such as hospital or skilled nursing facilities to the home setting without interruption while providing education to nurture PIHC’s ties with the rest of the health care system.

- Receives screens and appropriately disseminates information related to the coordination of patient care and program effectiveness. Gathers all necessary information including Home Health Face to Face documentation for CMS compliance. Completes data entry of patient demographic information, verifies payer coverage and coordinates logistics and information transfer. Provides the communication link between staff, clients, families, the medical community, other internal departments and other community agencies.
- Makes regularly scheduled visits to hospitals, as well as elder care, skilled nursing and rehabilitation facilities, to explain PHC policies and admission criteria to patients and responsible family members. Meetings with patients and family members and help at their inpatient facilities prior to discharge, when possible, to facilitate a smooth transition to home care services.
- Assists in establishing a definitive home care plan prior to discharge, including assessment of the appropriateness of requested services, medical supplies and appliances.
- Obtains appropriate and applicable client information to allow establishment of home care services. This includes information such as physician signatures, patient education documents, histories and physicals, all of which will facilitate establishment of the plan of care.
- Accepts and evaluates the appropriateness of verbal or written orders directly from physicians, hospitals and social service agencies.
- Obtains initial reimbursement authorization by providing pricing and billing information in accordance with established guidelines. Documents initial insurance verification prior to starts of care.
- Processes clinical data including but not limited to computer entry of referral data, medication information, and faxed orders for home health and hospice services.
- Receives and responds to incoming referrals and evaluates them for reimbursable care.
- Serves as a resource to hospital units, skilled nursing facilities, physicians’ offices and elder care facilities concerning PHC programs and policies. Meets with referral sources and their office staff to market PHC services.
- Works with program clinical managers and team assistants to cooperate scheduling and ensure timely service delivery.
- Serves as a resource to recommend services to agency staff.
- Handles inquiry calls regarding general program information. Serves as a resource for PHC services.
- Assists in provision and coordination of new hire orientation.
- Performs other duties as assigned by supervisor.

- Current LPN or RN license in the State of Montana
- Valid driver’s license, reliable transportation, and appropriate auto insurance
- Two years of previous acute care experience or one year of recent experience in a home\-based setting are preferred.
- Business Development or prior liaison experience preferred.
