    Role Summary

    The Remote Advanced Practice Provider (APP) plays a crucial role in ensuring the success of PocketRN’s dementia care (GUIDE) program. Working closely with the Chief Medical Officer, provider liaisons, clinical teams, care navigators, and patients, the APP will review clinical documentation, complete CMS\-required attestations, and support timely, high\-quality dementia care. This role helps ensure that patients and caregivers have access to the services and support they qualify for.

    Key Responsibilities

    ### Clinical Responsibilities

  • Review and sign off on documentation from care navigators and nurses in a timely manner.
  • Enter relevant charges into the electronic medical record (Athena) for services provided.
  • Complete provider attestations as required for the dementia care program(s).
  • Provide oversight to care navigators and nurses supporting program beneficiaries.
  • Respond to escalations from clinical teams and care navigators as needed.
  • When appropriate, evaluate and diagnose patients with dementia when extenuating circumstances prevent assessment through their existing care team.
  • Communicate clearly and frequently with patients, caregivers, PocketRN staff, and partners.
  • Follow escalation protocols for urgent scenarios and ensure timely communication with relevant parties.
  • Maintain accurate, timely, and thorough documentation of patient data and clinical decisions.
  • Maintain strict adherence to HIPAA and privacy standards in a virtual care environment.
  • ### Telehealth \& Administrative Responsibilities

  • Maintain an active, unencumbered APRN license in all states where care is delivered.
  • Review and sign off on completed comprehensive assessments, medication reconciliations, and required attestations within 7 days of the patient visit, when serving as the billing provider. This applies to patient visits conducted by nursing staff that requires provider review for completion and billing purposes.
  • Participate in patient, caregiver, and staff education and coaching when requested.
  • Conduct remote calls and video visits on PocketRN, including urgent escalation visits when needed. Appropriate documentation must be completed in 24 hours.
  • Complete all required training to stay current with PocketRN clinical protocols and workflows.
  • Complete all required clinical competency assessments and orientation checkoffs within 30 days of the start date, demonstrating readiness to independently manage provider visits, and meet organizational clinical standards.
  • Assist with the creation of clinical content and training materials as requested.
  • Perform administrative tasks as needed to support program operations.
  • Cooperate with billing, documentation, credentialing, payor enrollment, and compliance requirements.
  • Participate in quality assurance, peer review, and performance improvement initiatives.
  • Prepare accurate and timely electronic medical record entries.
  • Notify PocketRN within 72 hours of any event that may affect licensure, credentialing, or professional standing.
  • Qualifications

    ### Required

    * Dementia Proficient Provider, meeting at least one CMS criterion:

    + At least 25% of patient panel within the past 5 years comprised of adults with cognitive impairment (including dementia), or

    + At least 25% of patient panel within the past 5 years aged 65 or older, or

    + Specialty in neurology, psychiatry, geriatrics, geriatric psychiatry, behavioral neurology, or geriatric neurology.

    * Education: Graduate of an accredited school of nursing.

    * Experience: Minimum 2 years of direct patient care as an Advanced Practice Provider.

    * Licensure: Current APRN license(s); New Mexico or Alaska

    * Communication: Strong verbal and written communication skills; proficiency with asynchronous communication in a remote environment.

    * Professionalism: Courteous, empathetic, and patient\-centered communication.

    * Technical Skills: Proficiency with electronic health records (Athena preferred), Google Workspace, and telehealth tools.

    * Home Setup:

    + Private, quiet location for HIPAA\-sensitive calls

    + Reliable computer (no Chromebooks)

    + Internet speed of at least 50 Mbps

    ### Preferred

  • Experience working in virtual care or startup environments.
  • Experience supporting dementia or geriatric populations.
