In Reach Therapy is a community\-based speech and occupational therapy practice serving children and families across Oregon. Our clinicians provide therapy in homes, schools, and community settings, meeting families where they are. We serve approximately 17 providers and accept most major commercial insurance plans as well as Oregon Health Plan Medicaid. We are looking for a Billing \& Office Manager to lead our administrative and billing operations. This is a leadership role for someone who understands medical billing deeply, can manage people with clarity and accountability, and is not afraid to walk into a mess and build order out of it. We're growing and modernizing, building custom technology to streamline our billing and authorization workflows, with more improvements on the way. We're looking for a manager who's excited to be part of that evolution, who isn't rattled by a busy season, and who genuinely enjoys the challenge of tightening up processes and making things run smoother. This is not an entry\-level billing role. We need someone with advanced, hands\-on knowledge of the full claim submission and remittance cycle, including clearinghouse operations. What You'll Do: Team Leadership \- Supervise four office staff: Front Office Representative, Office Assistant, Billing Specialist, and Authorization Specialist \- Set clear expectations, provide training and support, and hold the team accountable for accuracy and follow\-through \- Review and approve work from newer team members until they've demonstrated consistent accuracy \- Build a culture of ownership, people who catch problems, not people who let them slide Billing Operations \- Oversee the full revenue cycle: claims submission (837P), remittance processing (835\), payment posting, denial management, secondary billing, and patient invoicing \- Understand clearinghouse operations end to end, including claim acknowledgment, rejection reports (999/277\), and ERA delivery \- Monitor claims aging and denial trends, and direct the team to prioritize by recoverable dollars \- Investigate and resolve complex claim issues, including payer\-specific problems, authorization discrepancies, coordination of benefits, and clearinghouse delivery failures \- Ensure Oregon Health Plan / Medicaid secondary billing is submitted accurately and timely \- Maintain and strengthen relationships with payer representatives Authorization Oversight \- Ensure authorization tracking is accurate and renewal requests go out ahead of expiration \- Support the authorization specialist in resolving denials and payer\-specific requirements Office Operations \- Oversee client intake workflows, records requests, and front office responsiveness \- Ensure client files are set up accurately for billing from day one \- Identify and fix broken workflows before they become financial problems What We're Looking For: \- Advanced, demonstrated knowledge of medical billing, including the complete claim lifecycle from submission through remittance and payment posting \- Fluent understanding of 837P claim files, 835 remittance files, CARC/RARC denial codes, and clearinghouse processes \- Prior experience managing a billing or office team \- Deep understanding of CPT/ICD\-10 coding, ERA/EOB interpretation, denial resolution, and coordination of benefits \- Experience with Medicaid/OHP billing REQUIRED \- Someone who can read a claim, understand what actually happened, and know what to do about it, not just follow a script \- A person who takes ownership of problems \- Comfortable with technology and eager to help shape new billing systems as we build them \- Strong communicator who can manage a remote team with clarity and consistency \- HIGH tech proficiency\- if you cannot make a V\-lookup in Excel, you will not be right for this position.
