Hybrid schedule, two days per week in office Schedule: 9:00am\-5:00pm Monday to Friday Position Summary The Case Manager delivers homeless prevention case management for households at risk of losing their housing across Chicago. Working within Street Samaritans' State Homeless Prevention program, the Case Manager receives referrals, assesses eligibility and need, connects households to financial assistance and supportive resources, and carries each case through to a timely resolution. The role combines direct client engagement with disciplined documentation in HMIS and Salesforce, and is central to meeting the performance and compliance standards set by All Chicago and IDHS. Key Responsibilities:Intake and Referrals: Accept and manage an average of ten referrals per week, the majority routed through the Homeless Prevention Call Center, with a limited share of walk\-in clients. Contact every referred household within three business days, following the contact protocol established by All Chicago. Confirm that funded households meet core eligibility criteria and risk indicators in accordance with program policies. Case Management and Supportive Services: Assess each household for resources including LIHEAP, SNAP, and other benefits, and assist clients in applying as needed. Guide households through the financial assistance application, working toward the program target of distributing rental and related assistance to households served. Notify households denied funding of the reason for denial and their right to appeal, and document the outcome. Follow up with every household ninety days after funding approval and report outcomes to All Chicago. Maintain a language access approach so households who speak languages other than English can be served effectively. Performance and Compliance: Move cases from referral to funding approval within a median of thirty days or fewer, and resolve cases within thirty days of referral on average. Review and resolve applications placed On Hold within thirty days, keeping on\-hold cases to a minimum each month. Adhere to all program policies and procedures, including the Continuum of Care Community Standards. Respond to communications from All Chicago within two business days, and attend required meetings and trainings. Data Quality and Documentation: Enter and maintain a complete HMIS enrollment for every household served, with all required data elements and an updated need status. Complete an HMIS Consent Form for Data Sharing for each enrolled household. Upload all required documentation into Salesforce when completing requests for assistance, and retain client records consistent with funder retention requirements. Support accurate, timely data entry to meet All Chicago and IDHS reporting obligations. Qualifications:Required: Bachelor's degree in social work, human services, or a related field, or an equivalent combination of education and relevant experience. Experience in case management, housing, benefits navigation, or direct service with people experiencing housing instability. Strong organizational skills and the ability to manage a steady caseload against deadlines and performance benchmarks. Comfort with data systems and a commitment to accurate, timely documentation. Clear written and verbal communication, and the ability to work respectfully with clients from a range of backgrounds and circumstances. Preferred: Working knowledge of HMIS, Salesforce, and Chicago homeless services or the Continuum of Care system. Familiarity with LIHEAP, SNAP, and local rental assistance and benefit programs. Proficiency in a language other than English spoken by the communities Street Samaritans serves. Working Conditions: Hybrid schedule with two days per week in the office and remote work on remaining days, subject to program and client needs. Standard business hours, Monday to Friday, with availability to accept referrals during program hours. * Occasional travel within Chicago for client meetings, community partners, or required trainings.
