Overview WeCare24 is a dynamic, fast\-growing provider of aging\-in\-place technology and remote care management services that help seniors age with optimal quality of life. We provide RTM, RPM, CCM, and BHI services to patients we are responsible for. The Care Management Assistant Team Leader supports WeCare24’s mission of delivering dependable, compassionate in‑home senior care by coordinating daily care‑team operations and managing improvement projects that strengthen service quality, communication, and organizational efficiency. This role ensures caregivers, clients, families, and leadership stay aligned, and that both routine care activities and strategic initiatives run smoothly as the company grows. Core Responsibilities Training, scheduling, evaluation and support of your Care Team members in the completion of remote care management and related services. Maintaining timely, complete, and accurate records of the team’s activities in accordance with Company guidelines. Acting as liaison with the treating physicians and other representatives of Company’s clients, including participation in quality assurance and compliance matters. Reporting to and coordinating with Company’s leadership in operations and strategic planning. Required Skills \& Qualifications Strong health care experience to be used when working with the care coordination of clients Strong organizational and multitasking skills to balance daily coordination with project timelines. Administrative experience and familiarity with the Microsoft Office Suite (Teams, Excel, Planner, SharePoint) is important. Excellent communication skills for interacting with staff, physician office staff, caregivers, clients, families, and leadership. Ability to analyze problems, propose solutions, and support change management. Understanding of home‑care operations, caregiver workflows, and senior‑care needs. Compassionate, patient‑centered mindset aligned with WeCare24’s mission. Calm under pressure, especially when managing urgent schedule changes or client needs. Detail‑oriented with strong follow‑through. Comfortable coordinating across multiple priorities in a fast‑growing environment. Education/Experience Associate or bachelor’s degree in nursing, business, or a related field Minimum RN licensed Minimum two years of experience in telehealth or care management services for chronic care management, remote patient monitoring, etc. Job Type: Part\-time Pay: $5,000\.00 \- $6,000\.00 per month Application Question(s): Do you have experience managing clinical staff, including scheduling, oversight, administering care plans, and documenting results?
